Our providers help patients navigate the health care system, including specialist, hospital care coordination, and follow-up care.
Dr. Noah Hammond, MD
Dr. Noah Hammond is a dedicated primary care physician and Medical Director at CHP, with a strong background in internal medicine. Originally from Nebraska City, Dr. Hammond earned his undergraduate degree in Biotechnology from the University of Nebraska Omaha. He then pursued his medical education at the University of Nebraska Medical Center, where he earned his Doctorate of Medicine and completed his residency in Internal Medicine.
In addition to his Medical Director in primary care role, Dr. Hammond provides inpatient care at Bryan Health in Lincoln, contributing his expertise to the care of hospitalized patients.
Outside of his medical practice, Dr. Hammond values spending time with his wife and five young children. The Hammond family are passionate supporters of the Nebraska Cornhuskers and enjoy attending games and supporting their team together.
Ashley Synowicki, DNP
Ashley started her health care career by attending Methodist College where she earned her Bachelors of Science in Nursing. While working at Methodist Hospital, she went back to school for her Doctorate of Nursing Practice (Family Nurse Practitioner) from Creighton University. She comes to us with years of advanced practice knowledge, expertise, and skills. Ashley is extremely versed in all thing's health and wellness. She prides herself on being incredibly organized, precise, and emerges herself in the latest research to help better her practice.
When not working incredibly hard, you can usually find Ashley on a walk with her husband and puppy, spending time with her family, or watching the Minnesota Vikings. Ashley enjoys family practice medicine because she believes that whole self-wellness is the key to living a long, happy, healthy life.
Deb Bussey, APRN
Deb Bussey is an Adult Gerontology Primary Care Nurse Practitioner, board certified by the ANCC. She began her nursing career over 33 years ago obtaining a Bachelor of Science in Nursing from UNMC. She has worked in a variety of healthcare settings including the past 10 years as a wound, ostomy and continence nurse. She strongly believes in lifelong learning and graduated in 2023 with a Master of Science Nurse Practitioner degree. She enjoys the challenges presented in working with the adult and geriatric population.
Deb resides in Lincoln with her husband and has 3 adult children living in the Lincoln and Omaha area. She enjoys spending time with family, outdoor activities, gardening, exercising, traveling, and caring for her dogs and cats.
Christabel Alphonso, FNP/PMHNP
Christabel Alphonso is a Board-Certified Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner. She is originally from Nigeria and moved to the United States in 1999. She started her career in nursing as an LPN in 2006 and has worked in different capacities, from Home Health Nurse to Rehab to Med-Surg Nurse. She obtained her Doctor of Nursing Practice degree (FNP track) from Creighton University in Omaha, NE, in 2018 and completed the Psychiatric Mental Health Nurse Practitioner Certification Program from Liberty University in VA in 2023. She currently serves as an Active-Duty Air Force FNP and recently joined CHP as a part-time FNP/PMHNP. She has been married to Dala Alphonso for 27 years and is blessed with 3 daughters - Tehillah 25, Davina 21, Alexea 13, and a stepson Carlton 26. Her favorite thing to do is lounge on the couch and hang out with friends and family. She also loves traveling (but hates the packing and unpacking part of this).
Lana Ekberg-Wood, FNP
Lana Wood is a native of Wakefield, Nebraska. After graduating from Wayne State College with a degree in Psychology, she attended UNMC and received her BSN going on to work inpatient Cardiology and in Home Health Care. In 2006, she enrolled at Clarkson College in Omaha where she earned her Master’s Degree in Nursing as a Family Nurse Practitioner. Lana has been a Nurse Practitioner in Omaha and Lincoln since 2009, specializing in Urgent Care, Home Health Care and Aesthetics. She is married and has one daughter.
Renee Schmidt, NP
Renee Schmidt is a dedicated Family Practice Nurse Practitioner who earned her Master’s degree in Family Nursing Practice from Clarkson College in Omaha in April 2019. With a diverse background, Renee has extensive experience in orthopedics, cardiology, geriatric and acute rehabilitation, long-term primary care, hospice, and women’s health. Prior to becoming a Nurse Practitioner, she worked as a scientist, bringing a unique perspective to her clinical practice.
As a Nurse Practitioner, Renee is passionate about providing individualized care and building strong, personal relationships with her patients. She values the opportunity to educate and empower her patients, helping them make informed decisions about their health and well-being.
Outside of her professional life, Renee enjoys spending quality time with her family. When she is not at her kids sporting events she enjoys reading, boating, traveling and going to concerts
Jenny L. Hackwell
APRN, FNP-BC, CWCN
Jenny is a part-time Wound Care Nurse Practitioner at Community Healthcare Partners, bringing with her a wealth of experience in wound care. She graduated from Purdue University in January 2021 with her APRN and has had a diverse career in nursing. From 2001 to 2021, Jenny worked as a Licensed and Registered Nurse in various settings, including hospital nursing, home health nursing, where she also served as the Director of Nursing, and as an Infusion Clinical Liaison. Since 2021, Jenny has been with Nebraska Medicine in Omaha, specializing as a Wound Care and Plastic Surgery Nurse Practitioner. She has a strong passion for patient-centered care and enjoys collaborating with healthcare professionals to deliver exceptional wound management and support throughout the surgical process.
In her personal time, Jenny enjoys spending time with her family and friends.
Cindy Kathman, NP
Cindy completed her BSN in 2000 through Creighton University and her MSN through Clarkson College in 2017. She worked to formalize the palliative care program at Mary Lanning Health Care in Hastings. As the palliative care nurse practitioner, she saw patients at the hospital, nursing homes, ALFs, and in their homes within a 60-mile radius of Hastings. She has also been the in-house provider at Edgewood Assisted Living since 2020.
Cindy resides in Juniata, Nebraska, with her husband. They have three children and four grandchildren. In her spare time, she enjoys spending time with her grandkids and gardening. She commutes to Lincoln two days a week.
Andrew "Andy" Lundstrom, APRN-BC, AGACNP
"Andy" Lundstrom, APRN-BC, AGACNP, is a nurse practitioner with significant experience in acute care internal medicine. He received his bachelor’s degree at the Bryan College of Health Sciences in Lincoln, NE, majoring in nursing sciences with a minor in healthcare management. He completed his master's degree at the University of Nebraska Medical Center’s College of Nursing in Omaha, NE.
Andrew is board-certified as an adult gerontological acute care nurse practitioner. Prior to joining Community Healthcare Partners, he worked with Inpatient Physician Associates on the hospitalist team at Bryan Medical Center in Lincoln, NE. Before completing his master's, he worked for eight years in the cardiovascular ICU at Bryan Medical Center and as a ventricular assist device coordinator for Bryan Heart. He also served as a graduate assistant and was a clinical instructor for undergraduate nursing students.
Cory Shield, DO
Internal Medicine Physician
Dr. Cory Shield is an experienced internal medicine physician with over eight years of clinical experience. He grew up in Holdrege, Nebraska, and has always been committed to serving the community and making a meaningful impact in the field of healthcare.
Dr. Shield pursued his undergraduate studies at the University of Nebraska at Kearney, where he developed a strong foundation in the sciences and a passion for medicine. He then went on to earn his medical degree from the Kansas City University of Medicine and Biosciences, where he gained a comprehensive understanding of medical theory and patient care.
After medical school, Dr. Shield completed his residency in Internal Medicine at the CHI Internal Medicine Residency Program in Des Moines, Iowa. During his training, he developed a particular interest in general internal medicine and patient-centered care, skills he has honed over the years in his clinical practice.
Now with over eight years of experience, Dr. Shield is dedicated to providing high-quality care to his patients, with a focus on building long-term relationships and managing complex health conditions. He is committed to continuous learning and staying up to date with the latest advances in medicine.
Outside of his medical practice, Dr. Shield enjoys spending quality time with his wife and three boys. He has a passion for reading, golf, and running, activities that help him maintain balance and well-being in his personal life.

Taylor Harding

Taylor Harding is an Adult Gerontology Primary Care Nurse Practitioner. She began her nursing career as a CNA/Med Aide while going through nursing school. She graduated from Nebraska Methodist College with her Bachelor of Science in Nursing. While working at Methodist Hospital, she went back to school for her Doctor of Nursing Practice. She enjoys taking care of the Older Adult Population and providing them with quality and effective care.
Taylor lives in Elkhorn with her husband and two kids. She enjoys spending time with her family and friends.
Dr. Alisa Nollendorfs, MD
Nicki Hunt, Office Triage LPN
Nicki is our triage nurse at Community Healthcare Partners. She has been a Licensed Practical Nurse since 2013. She knew, from the beginning of her nursing career, a triage position was where she wanted to be as she is passionate about being a voice for patients and an advocate for them when in need. Her strong desire to show empathy and provide reassurance to patients is what gratifies her. She was born and raised in Lincoln, Ne. She has twin 17-year-old boys and is an avid animal lover. She has two rescue cats, Sophie and Winter, and enjoys spending time with family and vacationing together.
Tasha Hamilton LPN
Tasha is an experienced nurse with a diverse background in patient care, earned through years of dedication and professional development. After completing her LPN certification at the College of Saint Mary in 2011, she has worked in a variety of healthcare settings, including adult pulmonary care, long-term care, home health, and nursing management. Her broad clinical experience has shaped her approach to nursing, with a strong focus on patient advocacy, compassion, and meeting the unique needs of each individual she serves.
With a deep passion for improving patient outcomes, Tasha is committed to providing high-quality, personalized care while empowering patients and families through education and support. Originally from Omaha, NE, she enjoys spending quality time with her 16-year-old daughter and values the importance of family.
Michelle Berry, Director of Marketing
Michelle brings a wealth of experience and knowledge from the home healthcare and sales and marketing industries to our team. In 2008, she graduated from the University of Phoenix with a MBA. Michelle enjoys networking and meeting new people in order to spread the word about Community Healthcare Partners.
In Michelle’s spare time, she enjoys spending time with her family and friends. She also volunteers for a non-profit organization that is dear to her heart.
Ella Gibbons, Phlebotomist
Ella was born and raised in Lincoln, Nebraska. She graduated from Lincoln East High School in 2023. Ella attended a TCA program through Southeast Community College, where she completed her phlebotomy course. Ella currently works at Physician’s Lab and for Community Healthcare Partners. Ella loves to cook and enjoys attending local farmers' markets.
Becky Schultze, Office Administrator
Becky is a part-owner and the Office Administrator of Community Healthcare Partners (CHP). With an extensive background in property insurance, Becky led a team of employees at State Farm for 20 years, honing her leadership and operational skills.
After transitioning out of the insurance industry, she found her calling in the Primary and Women's Healthcare sector, where she served as an office administrator for a small local practice in Nebraska.
In 2021, Becky played a pivotal role in the development of CHP, and today, she wears many hats in managing the administrative aspects of the primary care practice.
Outside of her professional endeavors, Becky enjoys spending time with her family and friends, with activities such as camping, traveling, game nights, and enjoying the ocean.
Samantha Murdock, Scheduling/Office Support
Samantha is a graduate of Palmyra High School and currently serves as a full-time Scheduler and Medical Records Specialist at Community Healthcare Partners. With a strong dedication to patient care, she consistently exceeds expectations in her role, demonstrating exceptional skills in patient communication, organization, and attention to detail. Samantha’s ability to manage multiple tasks efficiently while ensuring a positive patient experience has made her a valuable asset to the team. Outside of her professional responsibilities, she enjoys spending quality time with her family and attending racing events.
Wes Rice, Human Resources
Wes Rice is the HR Manager at Elite Professionals, a role he has held for over 18 years. Starting his career with the company as an office assistant, Wes has steadily worked his way up, expanding his expertise in human resources. In his current role, he supports all local NE Promontory companies with their HR needs, ensuring each organization’s compliance and smooth operations.
Wes has a specialized knowledge in Emergency Preparedness and Disaster Policies and Procedures, ensuring that each company is fully up to date and equipped for any situation. His diverse skill set makes him a key resource in maintaining safety and regulatory standards for the companies he serves.
Outside of work, Wes is passionate about gardening, landscaping, and taking on home improvement projects. He and his husband enjoy spoiling their three dogs and spending quality time with them.
